This Performance Improvement Plan is being implemented to enable you to improve

your performance to successful levels. As your manager, I am fully committed to assist

you throughout this process; however, you are ultimately accountable for both your

efforts and results under this plan. I initially anticipate this plan will be in effect for

approximately. However, during the follow-up process we will continuously reassess this

timeframe based on your progress. An accelerated timeline may result if immediate

improvement efforts are not demonstrated.
